If you have a scanner, kindly scan your medical order so we can study the medical indication before issuing an appointment. Please scan both sides of your medical card and include a form of identification with photo ID to expedite the appointmente process. Frequently the medical plan requieres a preauthorization form with a series of stringent requirements among which may include if the study will require intravenous contrast injection, among various other details. ANATOMIC REGION: Each anatomic region constitutes a separate scan. For example, brain would be a separate study from an orbital study; cervical spine would be a separate study from a lumbar spine; the right knee would be a separate study from the left knee.

RADIOLOGIC MODALITY: It is important which imaging modality you required to be able to issue an appointment. For example, CT and CTA scans are performed in a different machine than that required for MRI, MRA and MRCP scans. Hybrid PET-CT scans are performed in a differente machine than MRI scans.

INTRAVENOUS CONTRAST: If your study will require intravenous contrast, it is very important that you provide us with some initial clinical history.

For CT scans with intravenous contrast:

  1. Are you diabetic? If you are diabetic you must indicate if you take any oral hypoglycemic medication that contains Metformin in which case it would need to be stopped before and following the intravenous contrast administration for the CT scan. Such medication must be discontinued with the authorization and supervision of the physician in charge of managing the diabetes. This physician may or may not be the doctor who requested your CT scan. This physician will likely provide you with a plan to control blood sugar during the time the medication is stopped. 
  2. Have you had an allergic reaction to the iodine based intravenous contrast (CT, IVP, DSA/catheterization), to the gadolinium based contrast (for MRI and MRA), any medication or seafood? Please detail the reaction and all medical intervention required to control it. If no medical intervention was required, please state so. We will prepare a prescription for premedication consisting of a steroid and an antihistamine agent to reduce the severity of potential adverse reaction to the intravenous contrast that should be started the day before your study. 
     
  3. Do you have asthma? When was your last attack and what was required to control it? We will prepare a prescription for premedication consisting of a steroid and an antihistamine agent to reduce the severity of potential adverse reaction to the intravenous contrast that should be started the day before your study. 
     
  4. Do you have kidney function problems? Please describe.

For MRI and MRA scans with paramagnetic intravenous contrast (gadolinium):

  1. Indicate if you have had an adverse reaction against iodianted intravenous contrast (prior CT, IVP, angiogram/catheterization). We will prepare a prescription for premedication consisting of a steroid and an antihistamine agent to reduce the severity of potential adverse reaction to the intravenous contrast that should be started the day before your study. 
     
  2. Indicate if you have had an adverse reaction against gadolinium based intravenous contrast (prior MRI or MRA). We will prepare a prescription for premedication consisting of a steroid and an antihistamine agent to reduce the severity of potential adverse reaction to the intravenous contrast that should be started the day before your study. 
     
  3. Indicate if you have renal function problems. Specifically provide us with your most recent GFR (glomerular filtration rate) value and its date.
